Team refresher training

The Health Foundation recognises that teams require ongoing support to embed and sustain self-management support in the long term. At Bridges we recognise that sustaining self-management support can be challenged by changes in staff and service configuration. Refresher team training is an opportunity for teams previously trained by Bridges to regroup and refresh, as well as introducing new team members to the approach.

 

What is the aim of refresher team training?

  • To enable a team to rebuild, evaluate and sustain a shared approach to supporting self-management in every day practice.

Who is it suitable for?

  • Frontline staff in health and social care of any grade working in the same team, service or pathway.
  • Can be a mixture of staff previously Bridges trained and not previously Bridges trained.

What are the potential benefits for our service?

  • Teams who work in a more targeted, person-centred way can improve the quality of their interactions with patients and families, which in turn can see an improvement in measures of patient experience and satisfaction. Secondary service outcomes that can be expected include more efficient use of health and social care resources.

  • Patients and families who feel supported to self-manage feel greater confidence and control over their condition and daily lives. In turn this can bring about improvements in mood, functional ability, and perceived quality of life.

How big can the group be?

  • Minimum 10 participants, maximum 25 participants per workshop.
  • Please contact us if you would like to accommodate more or less participants.

How is training delivered?

  • The refresher workshop is delivered over one day (09.00 – 16.30).
  • The content will be bespoke to your team͛s needs so around the time of booking, you will be invited to talk to a member of the Bridges team on the telephone to understand your current situation and challenges.
  • Training is delivered at your place of work, usually by two Bridges trainers.  Bridges trainers often work alongside our Associate Trainers, who represent the multidisciplinary team and share their own experiences of successfully implementing Bridges in practice.

Access to unique self-management tools

  • Where applicable to your caseload, you will be supplied with our unique self-management tools for either stroke, TBI or long-term conditions, to use with patients and families.
  • It is possible to deliver training to teams with caseloads not covered by our existing tools, for example, falls rehabilitation services.

Learning outcomes

  • Evaluate the strengths and limitations of your service in supporting self-management.
  • Appreciate the enablers and barriers to sustaining a shared approach to self-management support in the longer-term.
  • Revisit the theoretical and evidence-base to the Bridges approach to self-management support.
  • Demonstrate practical methods of enhancing self-efficacy in individuals, thereby achieving greater control in their everyday lives.
  • Demonstrate greater awareness of different communication styles and their impact on supporting self-management.
  • Demonstrate ways of using the unique self-management tools with patients and families and across different clinical settings.
  • Understand ways of involving family, friends and carers in supporting self-management.
  • Develop strategies to evaluate and sustain self-management support in practice.

 

Support for implementation and evaluation

  • The Bridges team will guide you in ways to implement and evaluate new and enhanced self-management support strategies; Bridges is ideal as a service improvement project to test an innovative way to sustain an effective approach to self-management support.
