
Bridges Self-Management Practitioner Training
Bridges Self-Management Practitioner Training builds health and care professionals’ knowledge, skills and confidence to deliver effective personalised self-management support for everyone they work with.
This training increases participants’:
- Knowledge of Bridges core principles of personalised self-management support and Bridges’ underlying evidence base that supports its effectiveness
- Skills and tools to embed the core principles throughout clinical practice
- Confidence and capacity to work in this way with patients and in teams
- Underpinned by co-design and ongoing research
- Always co-delivered by facilitators with lived and clinical experience of long-term health conditions
- Fully contextualised to address and support your team and practitioner needs

Impact:
Bridges improves wellbeing of patients, staff, and teams. It streamlines ways of working, focussing on high quality interactions that get to the heart of what matters most and build self-efficacy.
People feel more in control, less isolated, and able to manage the day-to-day ups and downs of living with a health condition. Staff and teams report increased confidence, a reconnection to their job purpose, and improvements in job satisfaction. When effectively implemented and sustained, it results in system-wide improvement.
The Course:
- 4 highly interactive 2-hour online workshops delivered over 6 weeks, including small practical elements to try outside the sessions and a break to practice and reflect on learning so far
- A workshop handout that has key tools, theories and spaces for reflections and activities
- 8 CPD points on completion of case reflection
- 2 years’ of implementation and sustainability support
- Access to Bridges’ online Community of Practice, full of resources and tools
- Contextualised to the needs of participants and teams
Our training is most effective when groups of 12 can participate from the same team, and we also run mixed groups for individuals or small groups from different teams and disciplines to learn together. Support to sustain this way of working
On completion of the training, participants are invited to become a Bridges Champion: those that fly the flag for Bridges, act as agents of change within their service and facilitate support from Bridges for the wider team for two years after your course.
Champions gain access to a further 15 hours’ of CPD per year:
- Monthly peer coaching sessions facilitated by the Bridges team
- CPD accredited termly Champions learning events
- Online resources to help embed and sustain Bridges
- Champions newsletter sharing good practice from other Champions and news from Bridges
